Numerous studies have demonstrated that infertile couples have significantly higher rates of sexual dysfunction than healthy controls. Similarly, it has been established that those with secondary infertility have more sexual dysfunction compared with those with primary infertility. To date, however, no studies have stratified the sexual and psychological impact of infertility by demographic or clinical characteristics, including infertility etiology.

Methodology

This study is a cross-sectional analysis of women in infertile couples seeking treatment at academic or private infertility clinics. Basic demographic information was collected. Respondents were surveyed regarding sexual impact and perception of their infertility etiology. Multivariate regression analyses were used to identify factors independently associated with increased sexual impact.

A randomized study of whether oocyte maturation and pregnancy rate could be improved by human chronic gonadotrophin (HCG) priming before immature oocyte retrieval in unstimulated patients with polycystic ovarian syndrome.

Several authors have reported pregnancies following the transfer of embryos produced from immature oocytes derived from stimulated ovaries. However, the pregnancy rate reported is correspondingly low. It appears that the oocytes retrieved from small follicles in women undergoing ovarian stimulation respond to human chorionic gonadotrophin (HCG) which promotes the initiation of oocyte maturation. However, it remains unclear whether priming with HCG before immature oocyte retrieval in unstimulated ovaries could improve the rates and time course of oocyte maturation and fertilization, as well as the embryo quality or not? The present study will follow randomized study to to determine whether priming with HCG 36 h before immature oocyte retrieval alters the rates of oocyte maturation, fertilization and early embryo development in patients with PCOS or not?

Materials and methods

A total of 24 patients with PCOS underwent 24 will complete treatment cycles. All patients will be under 41 years of age (mean 35.4 ± 4.9 years) and present with irregular menstrual cycles, anovulation (determined by ultrasound; see below) and a minimum 2-year history of infertility. The patients with PCOS will be assigned either to be primed with 10 000 IU HCG (Profasi; Serono, Oakville, Ontario, Canada) (group A), or not primed (group B). In group A the HCG will be given s.c. to patients 36 h before oocyte retrieval. Transvaginal ultrasound-guided oocyte collection will be performed using a specially designed 17G single-lumen aspiration needle (K-OPS-1235-Wood, Cook, Australia) with an aspiration pressure of 7.5 kPa.
